The Federal Communications Commission’s Telecommunications Relay Services (TRS) rules require that you must provide your consent for your VRS provider to transmit your registered information to the TRS User Registration Database (TRS-URD). Your registered information to be transmitted to the TRS-URD includes the following: full name; full residential address; ten-digit telephone number assigned in the TRS numbering directory; last four digits of the social security number or Tribal Identification number, if you are a member of a Tribal nation and do not have a social security number; date of birth; Registered Location; VRS provider name and dates of service initiation and termination; a digital copy of the user's self-certification of eligibility for VRS and the date obtained by the provider; the date on which the user's identification was verified; and the date on which the registered Internet-based TRS user last placed a point-to-point or relay call.
Your registered information must be transmitted to the TRS URD to ensure proper administration of the TRS program. Your failure to provide your consent to the transmittal of your information to TRS-URD will result in you being denied the use of VRS.
